The “Man-in-the-barrel syndrome” makes reference to a picture of bilateral brachial muscular weakness, of proximal predominance, that incapacitates the patients in the execution of functional activities related with the elevation of the upper limbs. We describe the case of a patient, 55 years, male, that has approximately 4 years initiated neurogenic picture of compatible characteristics with such syndrome. The Barthel Index was used to point the functional damages presented by the individual. We call the attention for this presentation and alert for diverse involved etiologies in these cases.
