Introduction. Hearing loss due to aging is progressive, symmetrical, bilateral and sensorineural, negatively affecting social participation, mental health and even occupational issues. Objective. To assess the health evidence on the relationship between hearing loss and cognitive decline in the elderly. Method. A systematic review (PRSMA) was conducted in Medline, LILACS, Scopus, ScienceDirect databases, manual search and gray literature, selecting studies published between 2010 and May 2020. The articles were selected by two independent researchers. Data were extracted regarding the main characteristics of the studies, including demographic characteristics, type of study, evaluations performed, components evaluated and results obtained. In addition, the methodological quality of the studies included in the systematic review was evaluated. Results. Out of the 218 studies identified in the databases, only five were included in the systematic review. Regarding the methodological quality of the articles included, the average obtained was 12 points, being categorized as high quality. Conclusion. Most studies showed an association between hearing loss and cognitive decline, highlighting that auditory rehabilitation is an important action to prevent the onset of dementia. The relationship between hearing loss and cognitive decline, as well as a possible causal path, becomes essential for the planning of public policies in search of an active and healthy aging of the population.
