Os autores abordam as dificuldades encontradas para estabelecer um diagnóstico diferencial entre os sintomas da síndrome pósconcussional e os sintomas de reação aguda a estresse e a transtorno de estresse póstraumático ocorridos após acidentes automobilísticos. Além de evidenciarem como os sintomas dessas síndromes ou transtornos assemelham-se e poucos são distintivos pelos critérios diagnósticos da CID-10, do histórico e dos exames laboratoriais que podem ser solicitados, apresentam um caso clínico em que esses sintomas mostram-se misturados, dificultando o discernimento clínico. Os resultados são discutidos e comentados de acordo com bibliografia internacional compulsada junto ao MEDLINE entre 1990 e 1999.
