Análise da hiper-extensão de joelho em pacientes hemiparéticos usando órtese para neutralização da fl exão plantar

Objetivo: Observar a eficácia da estabilização à 90º da articulação talocrural para diminuição da hiper-extensão de joelho de pacientes hemiparéticos durante a fase de médio apoio da marcha, bem como averiguar sua repercuss ão sobre a velocidade da marcha (VM). Método: Participaram do estudo sete pacientes hemiparéticos (seis masculinos e um feminino), todos apresentando hiper-extensão de joelho no médio apoio. Em uma avaliação foi mensurado o grau de espasticidade do tríceps sural. Uma órtese tornozelo-pé (AFO) foi utilizada para estabilizar a articulação talocrural. Imagens obtidas de perfil de marcadores articulares usando uma câmera digital registraram a angulação do joelho durante o médio apoio da marcha, ao longo de 10 metros, antes e depois do uso da órtese. Para mensuração e análise da angulação do joelho após registro em vídeo foi utilizado o programa Autocad, sendo analisada a angulação do joelho antes e após o uso da órtese. A VM também foi analisada. Resultados: Os resultados mostraram que houve redução significativa da hiper-extensão do joelho em seis dos sete pacientes (p<0,05), bem como melhora da velocidade da marcha na grande maioria. Não houve relação direta entre o grau de espasticidade e a hiper-extensão do joelho. Conclusão: A estabilização à 90º da articulação talocrural, utilizando a AFO rígida, mostrou-se eficaz para redução da hiper-extensão do joelho e melhora da VM.
