Introduction. Alexithymia refers to the inability to identify affective states and to reflect on and regulate emotions. It may be related to a neurofunctional alteration, a behavioral response and/or an organic disorder, as well as being associated with Alcohol Use Disorder (AUD). Objective. Synthesize existing data on the factors involved in the association between high rates of Alexithymia and AUD. Method. The research was carried out in March 2022 and used the Virtual Health Library (VHL), Scopus, ScienceDirect and Cochrane Library as databases. The descriptors "Alexithymia" AND "Alcohol" were searched. Results. After inclusion criteria, 10 results were found in the VHL, 18 in Scopus, 1 in ScienceDirect and 10 in the Cochrane Library. The studies found that the central factors were trauma and dissociation, stress, social anxiety and anxious-ambivalent and avoidant attachment. Conclusion. More research is still needed into neurobiological changes, such as increased activity in the posterior insula, anterior insula and central cingulate cortex in high levels of Alexithymia and its possible associations with AUD.
