Efficacy of cobalamin in the treatment of diabetic neuropathy: a scope review

Introduction. Given the increasing prevalence of diabetes in recent decades, it is necessary to develop therapies capable of reducing its complications. Objective. To investigate, through a scoping review, the efficacy of vitamin B12 in the treatment of diabetic neuropathy. Method. The study’s guiding question was formulated using the PCC (Population, Concept, Context) strategy, which guided the search for articles in the BVS, Embase, Scopus, and Web of Science databases. Articles were selected based on inclusion and exclusion criteria and subsequently subjected to the extraction process. Results. Five articles were selected, characterized as prospective clinical trials, including three open-label and two double-blind studies. These studies aimed to investigate the benefits and isolated efficacy of methylcobalamin (vitamin B12) in treating and improving symptoms associated with diabetic neuropathy. All articles demonstrated beneficial effects for this purpose, but there was divergence regarding the parameters used to determine these impacts. Two articles showed improvement in nerve conduction velocity, while pain reduction was also confirmed by two articles, although one classified this reduction as minimal. Additionally, two articles reported improvements in autonomic symptoms with methylcobalamin administration. Finally, there was discrepancy among studies regarding the effects of this therapy on the sural nerve. Conclusion. This review concludes that supplementing with vitamin B12 is beneficial for the treatment of diabetic neuropathy, highlighting it as a safe alternative without adverse effects resulting from this therapeutic approach.
