Relationship between ototoxicity and use of hydroxychloroquine in patients with systemic lupus erythematosus

Abstract

Introduction. For decades, hydroxychloroquine (HCQ) has been used to treat patients with Systemic Lupus Erythematosus (SLE) and an increasing number of studies have highlighted the potential protective effect of this medication through multiple mechanisms of action, offering individuals with SLE greater survival. Objective: To present scientific evidence based on a systematic literature review (PRISMA) on the possible ototoxic effects of hydroxychloroquine in patients with SLE. Method. For the selection of studies, the combination based on the Medical Subject Heading Terms (MeSH) was used. The Medline (Pubmed), LILACS, SCOPUS, WEB OF SCIENCE, SciELO and BIREME databases were used. Searches for scientific articles were conducted without restriction of language, period and location. Results. 315 articles with potential for inclusion were found, with only one article meeting the inclusion criteria and the guiding question that consisted of verifying whether the use of hydroxychloroquine causes effects on the auditory system of patients with Lupus. This study scored eleven (high quality) on the modified Pithon protocol for assessing their quality. Conclusion. There are still disagreements about the causal relationship between hearing loss and the use of HCQ in the treatment of SLE. Hearing disorders are frequently found in these patients, but there are still doubts as to the true etiology of these disorders.
