Introduction. Stroke is one of the main causes of disability in the world. Among the changes caused by the disease is hemiparesis, which in addition to causing motor changes, can cause changes in respiratory function, impacting the quality of life of patients. Objective. Evaluate the ventilatory muscle function in hemiparetic individuals due to stroke in the acute phase and its association with variables that express motor functional limitations. Method. This is a cross-sectional and descriptive study. Sample on demand, involving 30 patients, from April to September 2021. The assessments consisted of tests and specific scales for functionality, mobility and respiratory functions, through Timed Up and Go Test (TUGT), 5-repetition sit-to-stand test. (5xSTS), Peak Flow (PF) and manovacuometry. Results. A mean of 67.50±30.08 in MIP and MEP 75.33±29.48 was obtained, PEF in 335.67±151.25 were compared with the predicted values and lower limit of normality, not showing reduction in respiratory muscle strength, when compared by gender, it was observed that women had reduced MIP values and correlations considered weak with motor functions were obtained. Conclusion. The PEF was altered in both genders and women had lower values of MIP, significantly correlated with the motor functional limitations found.
