Cannabis medicinal en fibromialgia: un estudio de cohorte para una alternativa prometedora

Introducción. La fibromialgia se caracteriza por dolor musculoesquelético generalizado, alteraciones del sueño, fatiga, cefalea y síntomas cognitivos. El sistema endocannabinoide parece jugar un papel importante en la fisiopatología de esta enfermedad y el uso de fitocannabinoides se presenta como una posible opción terapéutica. Objetivo. Evaluar los efectos del uso de cannabinoides herbales, por vía oral y tópica, sobre síntomas específicos de la fibromialgia, además de evaluar la seguridad y los efectos adversos del tratamiento. Método. Se trata de un estudio de cohortes prospectivo, observacional. Se evaluó el resultado del uso, por vía oral y tópica, de aceite y ungüentos derivados de la planta Cannabis sp en los síntomas más frecuentes de Fibromialgia. Resultados. Se analizaron diez historias clínicas y el 100% de los pacientes eran del sexo femenino. Tras iniciar el tratamiento, se pudo observar una mejoría en la ansiedad y el insomnio, con una diferencia significante, así como una tendencia a la mejoría en los síntomas depresivos. También hubo una disminución en el uso de analgésicos antiinflamatorios no esteroideos, disminuciones en las puntuaciones de la escala de dolor y FIQ, y una tendencia hacia una disminución en el uso de opioides. El 71,4% de los participantes informaron efectos adversos leves y se resolvieron por completo dentro del primer mes de tratamiento. Conclusión. A pesar de la necesidad de más estudios, el tratamiento con Cannabis medicinal demostró ser efectivo y seguro para los pacientes con fibromialgia.
